Marla is a 42 year old Hispanic female accountant, that has complaints of trouble sleeping, feeling jumpy all the time, and unable to concentrate. With these symptoms it is causing her issues at work. There are several disorders that Marla could be suffering from but, it seems to be that one stands out from the rest. Post-Traumatic Disorder also known as PTSD, this is caused when a person experiences something traumatic such as a violent crime against them, a car accident, active military combat duty, the loss of a loved one, etc. Marla sat in my office nervous and unsure as if she was even in the right place. I asked her if she was okay and what brought her to see me. Marla told me of her symptoms and said she was unsure if she should even be here. I asked her if she has been to a family doctor to discuss her problems that she was having and she assured me that she had and came out with a clean bill of health. He doctor suggested to come see me and talked about what she was feeling and going through. With this and much reassurance from me telling her she did the right thing. I proceeded to ask her the following questions: Marla, what would you like to accomplish by being here? â€Å"I want to be able to feel normal again and to feel like myself again.† I think that is something that we can do with and I would like to ask you a little bit more about what you are feeling and maybe why you are feeling the way you do. Marla agrees. When did you start to notice the symptoms you are experiencing? â€Å"I started to really notice it about a month ago, my boss and some co-workers started to question me where I was not performing at my normal standards and coming into work late from not getting any sleep.† Is there any family history of depression, anxiety, or any mental illness? â€Å"Not that I am aware of†¦. As far as I know none of my family has ever had any issues mentally or been mentally ill.† She seemed kind of curios as to why I would ask such a thing, I ensured her that I am only asking to better help her and understand more ab out what she may be going through. Have there been any changes in your life recently? Good or bad? â€Å"Not that I can  think of.† Have you been a victim of a crime or served in the military on active duty before? â€Å"I have never been in the military or been the victim of a crime.† Have you had anything happen to you or somebody close to you? No, but I was a witness to a women walking in the parking lot with her husband and got hit by a car about six months ago.† Really, tell me more Marla. Well I was walking into work and an older women and her husband where walking hand in hand when a car driven by an elderly couple seemed to gain speed and struck the women where she rolled over the top of the car and landed several yards away from where she was. The elderly couple that was driving hit a pole which stopped their car. The driver had a heart attack and the passenger was hurt pretty bad but did not seem to be critical. When the ambulance and police showed up they called for a helicopter to take the women to the hospital that was struck, she was none responsive and just laid there twitching. There was blood everywhere and seemed to be pooling from the back of her head. I stayed there with the lady and tried to calm her husband that seemed lost and unsure of what to do. I was able to call his adult children to come to his aid and inform them of what happen. The EMT’s lifted her from the ground and laying there on the ground was a lock of her hair that was attached to part of her skull. They air lifted the women to a trauma center but died before they made it there. The couple in the car also passed away from injuries from the accident as well. I will never forget the look on the husbands face as he stood there helpless crying for his wife and pleading for them to help her. Well Marla I would say that could be pretty traumatic to see, have you talked to anybody about this? Not until now†¦ I did not think I would need to. Have you had any thoughts about this lately or think about it at all? Sometimes I do, I still cannot walk where she was lying on the ground I still see her laying there in my mind and them lifting her up with the piece of skull and hair laying there. I guess I was okay until I saw that. Marla I would have to say that you are suffering from Post-Traumatic Stress Disorder. What is Posttraumatic Stress Disorder? Why would I now be having or suffering from Posttraumatic Stress Disorder now 5 months later? PTSD is not always an instant onset of symptoms. It can come out days, months, or ever years later. Let’s talk more about this PTSD so you better understand it. Posttraumatic Stress Disorder: Posttraumatic Stress Disorder can occur in some individuals who have been exposed to a life threatening situation, or a situation which they have been witness in which another person is exposed to danger. The onset of symptoms with PTSD do not always happen immediately following the traumatic event, therefore the association between the symptoms and the initial event is often overlooked. A person with PTSD will be hyper-vigilant, and can react to an unsettling situation with physiological symptoms such as upset stomach, headache, or dizziness. People with PTSD can also suffer from depression or panic disorder, although this is not always the case. Avoiding stimuli which can trigger an anxious reaction is normal for the PTSD sufferer, and many become withdrawn from social interaction with friends and family members. Some may also have problems continuing an intimate relationship, and angry outbursts for no particular reason can cause problems with interpersonal relationships, as well as jeopardizing one’s employment. Treatment for PTSD can be relatively straight forward, although not always easy for the patient to endure. Being taught how to effectively relax is paramount, and once this goal has been achieved, the process of systematic desensitization can gradually take place. The desensitization process involves confronting the stimuli which cause the patient to have an anxious episode. Oftentimes, the stimuli from a particular location and for some it may be a certain situation which serves as an unconscious reminder of the traumatic event. The more a person is subjected to the offending location or situation, the less intense the fear becomes. This is achieved by implementing the relaxation strategies at the same time the stimuli are presented. The association between the relaxed state and the stimuli will eventually be achieved, disarming the stimuli and giving control back to the patient. In this situation Marla needs to see that there was nothing she could do. Being made aware of maladaptive responses to stimuli, patients are better able to identify which anxieties are due to the normal course of life, and which are patterns of irrational thinking related to their disorder.  According to DSM-IV, PTSD can occur at any age, to either gender, and the onset of symptoms can be delayed by months or even years (DSM-IV, 1994, p.426). According to Cognitive-Behavioral theorists, learned behaviors from conditioning can produce unsavory responses; both physically and psychologically. Unfortunately for the patient, these responses can occur whether it is their intent to react in such a way or not. In Marla’s case, the association with the accident she witnessed affected her life and her psychological and physiological having to walk by the certain spot where the accident took place as well as not speaking to or letting out how she felt or was feeling. Learned behaviors can be unlearned, just as new and more effective behaviors can be adopted. Marla, was correct in coming to talk to physiologist as to let out her feelings that she had pent up inside and to release the emotions that she had not expressed before now. Marla’s hyper vigilance and feelings of anxiousness can be alleviated and bought under control with practice, relaxation such as yoga, breathing technics, praying finding a solace in religion, keeping a diary of how she is feeling, and/or frequent exposure to the offending stimuli knowing that things do happen for a reason and there is nothing we can do to stop it, and in conjunction with Cognitive-Behavioral therapy, she may also benefit by taking an antidepressant (SSRI) such as Prozac, Zoloft or Paxil. This situation that I put in place for Marla is one that I actually experienced myself while I was at work and still to this day have flash backs of that day when I witnessed it and was placed in charge of handling the situation. It is not something I would ever want to see again or want to experience over. I did how ever take her death to heart and realized that my life had more meaning to it than what I was doing and decided to come back to school to earn my degree so I can help others and feel I make a difference to people. I will not let her death or the couple that died that was driving the car be in vein. I took this as a sign that I cannot waste my life doing something that does not make me happy, life is too short. Compare and contrast how the directors of ‘Titanic’ and ‘A Night to Remember’ portray the sinking of the Titanic

Using the factual accounts of the sinking of the Titanic, compare and contrast how the directors of ‘Titanic' and ‘A Night to Remember' portray the events that occurred. On Monday April the 14th 1912, brought an event which was to change thousands of lives and change the course of history. The ‘unsinkable' Titanic – the pride of the White Star fleet struck an iceberg and sank within hours causing a massive loss of life. This disaster has caught the imagination of many generations and has provided the source of many books and films both fact and fiction. The Titanic was on its maiden voyage to New York. It was the largest and greatest ship of its age, it was said to be like a floating city that carried 332 first class passengers in absolute luxury, 276 second class passengers and approximately 708 Steerage class or lower class passengers that suffered most casualties over the hours Titanic sank. The most famous films of the events surrounding the sinking of the Titanic are ‘A Night to Remember' by Roy Baker in 1958 which was based on the true story of Titanic, and most recently the 11 Academy award winning film ‘Titanic' by James Cameron in 1997 which is one of the most successful films of recent times. A Night to Remember' and ‘Titanic' approach the telling of the story in totally different ways, but have very similar themes running throughout. ‘A Night to Remember' is based on the book also called ‘A night to Remember' by Walter Lord. The film follows a chronological order of events with the central character of Lightholler the 2nd officer who survived the disaster. It focuses on him from when he boards the ship to when the ship is sinking. He is portrayed as a calm well respected leader, this is showed even more so when he shows courage and intelligence through his actions in keeping control of events and helping save lives. He plays a huge role in bringing the film together as he is someone who the watcher can focus on. The film highlights some of the mistakes that contributed to the disaster. It spends a large proportion of the time focusing on the radio room and how the operators where spending more time sending passengers personal business messages than listening to vital warnings from other ships of the danger of floating icebergs in the area. Throughout ‘A Night to Remember' the development of characters is very limited and many of them are focused on to show general scenes of passengers and crew and how they react to the events in different ways. In the opening scenes of ‘A Night to Remember' Baker, combines real footage of the launching of the titanic with acted scenes, he accomplishes this as he was so committed to telling the true story. However Cameron uses fake footage of the people waving goodbye to their loved ones, he fools us into believing it is real footage by changing the color contrast to a yellow to make it look old and historic. There are many stereotypes in the film, particularly the portrayal of the wealthy classes in first class, who appear to spend most of the film dressed in evening dress enjoying the splendor and luxury of the state rooms, and not realizing the severity of the situation when the ship strikes the iceberg. Many of them show little respect for the crew when asked to move to the lifeboats, and many resent having to wear life jackets. As the situation develops, and it becomes more apparent that the ships is really sinking then the mood changes and many develop an attitude of self protection. This is particularly shown by the Chairman of the White Star Line – Mr. Ismay who sneaks onto a lifeboat before many of the women and children have had a chance to leave the ship. This is noticed by Lightholler whose look of disgust shows his feelings towards his boss. Women in ‘A Night to Remember' are portrayed as thick and stupid as they make comments such as ‘You won't get back on board tomorrow without a pass' when the ship is about to sink to the bottom of the Atlantic. Throughout the film, a lot is shown of the class difference between the first class passengers, living in luxury, and the steerage class passengers who are accommodated in very basic accommodation below the waterline, with many sharing overcrowded rooms. Many of the gangways are locked during the sinking to prevent them getting on deck to have a chance of survival, as the crew know that the ship does not have enough lifeboats for everyone, and priority is being given to the first class passengers. Most of these steerage class passengers appear to be foreign immigrants to the USA, and thus of less value than the wealthy First Class passengers. Throughout the film the crew are shown to be professional and polite throughout, especially Lightholler, as the developing scenes of the film increase and when there is a rising tide of panic amongst the passengers and crew, Lightholler leadership qualities still stand out, as he is able to think in a calm and logical way, and overcoming many difficulties. However in ‘titanic' Cameron portrays him as a crew member who can only keep order by using weapons and treats ‘Order, order or I will shoot you all like dogs. ‘ The Captain of the Titanic, Captain Smith is portrayed in both films as a very stereotypical captain, with his white beard, and powerful presence, at no time during either film is it suggested that he is responsible for the disaster. In ‘Titanic' at the beginning he is shown as a dream maker and when the ship is sinking he is portrayed as lost and not in control. However in ‘A night to Remember' he is shown to be in control even when the ship is sinking as he gives the orders and in ‘Titanic' they are suggested to him by the crew. A Night to Remember' unlike ‘Titanic', spends some time on the two other ships in the area – The California which is only ten miles away, and could have reached the sinking Titanic quickly enough to save many of the passengers, but instead with its Captain asleep and the incompetent crew ignoring the frantic distress flare from the sinking ship, and turning off its radios. The Carpathia on the other hand is some sixty miles away when it hears the calls for help, and immediately starts racing towards the scene. The Captain is portrayed as efficient, and concerned in the way he prepares his ships to receive the casualties. The Titanic film makes no reference to the California, and The Carpathia only appears in the closing scenes of the film. The final scenes of ‘A Night to Remember', when the ship finally begins to sink beneath the surface is dramatic but without the benefit of computer graphics and animation available to Cameron, baker cannot possibly compete in terms of the drama that Cameron achieves in his modern film. In Cameron's film the ship is shown as breaking in two, with the stern rising vertically in the water before plummeting to the bottom. This was what actually happened according to eye witness accounts, but it is not portrayed like this in Bakers film. The sinking scenes in Cameron's film are some of the most dramatic and realistic in modern cinema. Cameron's ‘Titanic' makes a totally different approach to telling the story as it is based on fictional characters which are developed over a long period of time. It is basically a dramatic love story interwoven with the story of Titanic. The story develops between Rose, a rich girl who is forced into engagement to a multimillionaire steel baron – Cal Hockley. She feels trapped inside her own world causing her to feel like death is the only way out. However she is saved by Jack, a 3rd class passenger who makes a living out of pencil drawings. They soon fall into forbidden love as Rose's fiancie and her mother try to separate them several times. It uses flash backs from current times to the events in April 1912 through the eyes of a survivor of the disaster. The opening scenes of Titanic shows a salvage squad looking for a precious diamond that would be worth millions of pounds today. They finally find the safe which they believed it was kept in but they soon find out that the safe is empty apart from a drawing of a beautiful young girl wearing the diamond. This leads to a report on television which is seen by Rose who is now 104 years old as she recognises herself as the girl in the drawing. The salvage squad determined to find the diamond fly Rose out to the site where she recounts the events that lead to the sinking of Titanic. Throughout Titanic Cameron uses many different methods to represent the different classes by showing the contrast by focusing on the higher class to the middle class then to the lower class. In the opening scene Cameron focuses on the higher classes when the ship is being loaded with all the expensive cars, paintings and possessions and luggage. They are all dressed very smartly and are well presented. He then shows the different contrast between classes when moves the camera to show all the 3rd class passengers waiting to board. Many of them are in old ragged clothes with all there possessions in just one or two bags. He also shows how the lower classes are getting checked for fleas and lice before they board the ship. It is quite evident that many of the third class passengers are immigrants, heading for a new life in America. This class divide is repeatedly shown throughout the film, and it is more striking in Cameron's film than Baker's. After Jack talks Rose out of jumping overboard, Jack is invited to eat with Rose, her fianci, and the rest of their party, this is supposed to be a reward for saving Rose's life. It is a very grand dinner, in the first class dining room, and Cal and the rest of his party try to humiliate Jack, because he comes from a poor background and has no money ‘Mr. Dawson, well its amazing you can almost pass as a gentlemen'. Jack however does not seem to be affected, and it is clear that Rose is attracted to Jack, and finds him far more interesting than her self-centered fianci. Jack then leaves the dinner, shortly followed by Rose, and he takes her to ‘a real party' in steerage. Here the third class passengers are having a fantastic party, with dancing, and singing. Rose seems to be enjoying herself extremely as we also see her drinking pints of beer, which is a severe contrast to the expensive wine served from crystal classes, she has just had upstairs. It is clear that Rose is far more interested in Jack as a person, than Cal. This class divide is further shown when Rose and Jack, who by now are in love, are discovered by Cal and Cal's personal assistant/bodyguard, in Cal's state room. Jack has just done a pencil drawing of Rose lying down naked and wearing the expensive diamond that plays a large part of the film's plot. Jack and Rose run off, with Cal's bodyguard running after them. They run from the luxury of the stateroom with its own private walkway deck, probably the most expensive accommodation on the ship, through different levels of accommodation, through steerage class, with its overcrowded accommodation. Cameron even takes them through the engine room, which is represented, almost as a hell, with sweaty stokers shoveling coal to feed the boilers that are driving the ship. There is noise, fire, steam, and oil which together creates an immense contrast to the wealth and luxury of the staterooms at the top of the ship. Cameron's film has spent over half the film developing the main characters of Jack, Rose, and Cal. It is over one hour and forty minutes before the ship strikes the iceberg. From this tragic moment, there are many similarities between the two films. Of course Cameron's film, with its multimillion pound budget is far more effective, however the basic order of events remain the same, and it almost as though Cameron has made direct copies of some of the scenes from ‘A Night to Remember'. The scene when Mr. Andrews, the ships designer, who is on board, relates to the Captain that the ship will unavoidably sink ‘that's five compartments! She can stay afloat, with the first four compartments breached, but not five, not five, she will go down by the head and the water will spill over the tops of the baulk heads from one to the next' This is what Mr. Andrews says in ‘Titanic' and it is almost exactly identical to ‘A Night to remember'. There are many other examples of similarities from the point the ship strikes the iceberg. We see how at first passengers are unaware of what has happened, and how some are told (in both films), that the shuddering that was heard was ‘a propeller blade' being shed. We see in both films how the first class passengers are unwilling to follow the instructions of the crew to get into their life jackets. We see in both films how the third class passengers are locked down below decks, and the first class passengers are given priority in boarding the limited number of lifeboats. We see how there are examples of cowardice by men in both films, and how the chairman of the White Star Line, sneaks aboard a lifeboat before his turn. We also see how Cal tries bribing the crew to allow him aboard the lifeboat, when this fails he even uses child. This scene is not shown in Bakers film because Cal Hockley is a fictional character; however it does show how heartless Cal is and how men may have behaved during the event. In both films, even during the rising disaster, there are elements of humour, but these are very different in both films. In Baker's film we keep returning to a chef who is getting more and more drunk, as he drinks a whole bottle of whisky, not that amusing today, but forty years ago, it would have been. There is also a scene when some bell boys are told off for smoking in a public room, which is quite incredible when you think about the events unfolding around them. There is also another scene that is similar in both films, when the panicking third class passengers break down a barrier, and are accused of damaging ‘White Star property! ‘ This is said when the whole ship is about to sink to the bottom of the Atlantic! Cameron however is slightly more subtle with his humour. This is shown in the scene where Jack has been handcuffed to a pipe, and the only way that he can escape is if Rose can chop through the chain with a fire axe. As he is concerned that she might miss he asks her to practice on a cupboard, by hitting the cupboard, and then trying to hit the same spot for a second time, she misses by several feet, but as the situation is now frantic Jack urges her to chop through the chain. She manages it successfully, and they escape. Another example of humor is represented in the scene where the ship has sunk and jack says to Rose ‘I don't know about you, but I intend to write a strongly worded letter to the White Star fleet about all this. Cameron uses this comment to lighten the mood of the audience after witnessing such tragedy. Both films follow the same basic factual accounts of the sinking, and portray them in a similar way: the separation of children and women from their fathers; lack of life boats; priority given to 1st class passengers over 3rd class passengers to board the life boats; acts of cowardice, and the fact that the life boats did not return to help those in need. The main differences between the two representations are that ‘Titanic' follows a forbidden love story, and a ‘Night to Remember' focuses purely on the factual side of events. As ‘A Night to Remember' is presented in a way that it frequently fools us into thinking that it's a documentary, I believe this makes the story less interesting. In my opinion as a film ‘Titanic' is overall more successful in capturing the attention of the audience due to advantages of modern day special effects and the added striking love story.

This is clearly the nest step after universalizing elementary education. While the goal is laudable much work needs to be done before we are in a position to launch the Scheme for Universalisation of Access for Secondary Education (SUCCESS). Its details need to be quickly spelt out and discussed with States so that we are fully ready to launch it from 2008-09. We must not underestimate the complexity of this task as the principles for universalizing elementary education cannot be easily transferred to secondary education. The physical, financial, pedagogical and human resource needs are quite different. We also need to recognize the role currently being played by the private sector and policy design must factor this in. Detailed strategies and plans would need to be worked out rapidly for each state. Special attention would need to be paid to Districts with SC/ST/OBC/Minority concentrati

Comparison of Different Ibuprofen Dosage Forms Both ibuprofen gel and ibuprofen tablet produced similar efficacy in terms of pain relief in their respective groups. However, lower dose was administered in gel dosage form and fewer side effects were observed, in comparison to ibuprofen tablet. Patients consuming ibuprofen oral suspension reached the therapeutic effect at a quicker rate and were in that period for a longer duration compared to the other dosage forms. This suggested that it caused faster pain relief for a longer duration compared to ibuprofen tablets and ibuprofen chewable tablets. Introduction Ibuprofen (Figure 1) is chemically known as iso-butyl-propanoic-phenolic acid. It is a well known drug that belongs to a class of therapeutic agents known as non-steroidal anti inflammatory drug (NSAID). It possesses antipyretic (fever reducing) and anti-inflammatory (reduces inflammation) properties among others (i.e. anti-platelet effect). It is used in the treatment of pain and inflammation in rheumatic disease and other musculoskeletal disorders including minor aches and discomfort 78. A recent report showed ibuprofen had analgesic (pain-relieving) properties. This was shown by testing analgesic properties of ibuprofen on a mouse writhing and an inflamed rat foot. It showed there was similarity of analgesic activity in both species. However, ibuprofen failed to show analgesic activity in the normal foot of the rat or in the mouse hotplate test. Therefore it was established that ibuprofen is not a central, but a peripheral analgesic 13. Ibuprofen has fewer side effects than any other NSAID; however its anti-inflammatory properties are weaker than others. Doses of 1.6-2.4g are required daily for rheumatoid arthritis. It is unsuitable for conditions where inflammation is prominent such as acute group 8. Ibuprofen was discovered by Dr. Stewart Adams and his team (Figure 2) 3 in the 1950s, at Boots Company. The drug was patented in the 1960s and was initially marketed under the name Brufen. Initially the drug was tested on hangover, but the drug was launched for the treatment of rheumatoid arthritis in UK (1969) and USA (1974) 3. The mechanism of action of ibuprofen is not completely understood. However, ibuprofen is known to be a non-selective inhibitor of cyclooxygenase-1 and 2 (COX-1 and COX-2). COX is an enzyme that is involved in the production prostaglandins 8. Prostaglandins have an important role in the production of pain, inflammation and fever 13. Following administration of ibuprofen, it is rapidly absorbed and distributed throughout the whole body. The drug is eliminated through the kidneys 14. Ibuprofen is a derivative of phenylpropionic acid (Figure 3) 4. It contains a chiral centre (Carbon), therefore is non-superposable on its mirror image 2. This gives rise to enantiomers, resulting in two possible structures of ibuprofen. The importance of enantiomers is that all amino acids (apart from glycine) have a chiral centre. Amino acids are the fundamental blocks of enzymes and proteins in all forms of life including humans. Thus suggesting the human body is controlled by chiral molecules and effectively is a chiral environment. This results in different enantiomers having different effect on the body, including metabolism, toxicity to name a few 1. These enatiomers exist as (S) and (R)-enantiomers (Figure 4) 4. It was found that (S)-ibupofen was the active form in, in vitro and in vivo 2. Ibuprofen began to be marketed as a single enantiomer ((S)-isomer) so the selectivity and potency of ibuprofen could be improved. However, further in vivo testing led to the inactive (R)-ibuprofen to rapidly convert to active (S)-ibuprofen. Therefore the single enantiomer was scrapped and ibuprofen was to be marketed as a racemic mixture (50% of each enantiomers) 2, even now it is the same. Another reason was the likeliness of producing pure (S)-ibuprofen was too expensive on a large scale. The difference between both the enantiomers is the way the atoms are arranged and connected to the chiral centre. In the (S)-isomer the CH3 group is in the back, whereas in the (R)-isomer it is at the front. Since being launched it is widely available all over the world as over the counter (OTC), prescription only medicine (POM) and general sale list (GSL) products. In all countries over the world they are available under different names, formulations, strengths etc. In North America (Canada), ibuprofen is known as Motrin and Advil. In South America (Brazil) it is known as Alivium and Advil 5. Different countries have different guidelines and policies regarding selling and prescribing of ibuprofen. People are not just restricted to pharmacies but they can be obtained in supermarkets, general retailers etc. In many parts of the world including Australia and New Zealand, ibuprofen lysine is licensed for the same treatment as ibuprofen. Ibuprofen lysine is the salt form of ibuprofen and is the cationic form. As ibuprofen lysine has a net positive charge, it is more soluble than ibuprofen allowing the drug to be administered intravenously. This makes ibuprofen lysine to have a greater onset time of action than ibuprofen 15. Since 1977, World Health Organisation (WHO) has been producing a model list of essential medicines. This list is updated every 2 years and is known as List of WHO Essential Medicines. Ibuprofen is the only NSAID present in the list among other classes (opoids, antimetabolites etc) 6. It is classed as a core medicine, which means it is an essential drug for basic healthcare. The drugs listed are the most effective, safe and cost effective medicines for conditions that are a priority. This priority conditions are on the basis of present and future public health relevance. The drugs present on the list are recognised throughout the world. Ibuprofen tablets (200mg and 400mg) are present for the treatment of gout and rheumatoid arthritis. Also present is ibuprofen solution, which is used as an injection (5mg/ml). It is used in neonatal care for the treatment of mild to moderate pain 68. Discussion The most important role of a drug delivery system is to get the drug delivered to the site of action in sufficient amount and at the appropriate rate. This can be achieved by a predictable therapeutic response of the drug 11. However it must meet essential requirements, which include physical chemical stability, ability to be economically mass produced in a manner that assures the proper amount of active pharmaceutical ingredient (API) is present in each dosage and patient acceptability 9. It can be seen from Table 1 11, different dosage forms have different time of onset of action. Table 1: Shows the variation in time of onset of action for different dosage forms. It can be seen that intravenous injection is the most superior dosage form in terms of time of onset of action, as it takes seconds to produce an effect. Depot injections and implants take days to produce an effect. Tablets are one of the most popular ways of delivering a drug through the oral route. Tablets are solid preparations each containing a single and accurate dose of active pharmaceutical ingredient(s) (API). They are completed by compressing or compacting uniform volume of particles to a solid dose 10. There are different types of tablets available; they include effervescent/soluble, modified release etc. The aim of the modified release tablet is it enables the biopharmaceutical behaviour of the drug to be controlled. Many tablets are available that have coatings; these include film or sugar coating. All these tablets exist and are formed by the incorporation of different types of excipients 11. They vary in shape, colour, size, design etc 10. Tablets are popular for several reasons including the oral route to be the most safest and convenient route of administration. Compared to other dosage forms such as liquid, they are far superior in terms of chemical and physical stability. The procedure enables accurate and precise dosing of the API 11. These are a few among a large list. Drawbacks include elderly having difficulty swallowing, irritation and harm to the GIT, possibly leading to liver and kidney damage. Examples of ibuprofen tablets include Anadin, Ibuprofen tablets etc. Effervescent formulation is a type of immediate release tablet, as the tablet is dissolved and administered as a solution. This is the most common type of tablet 11. They are used to obtain rapid drug action. Effervescent tablets are placed into a glass of water, where carbon dioxide is liberated. The carbon dioxide is produced by a reaction in the water between a carbonate or bicarbonate and a weak acid. Once liberated, this helps tablet disintegration and drug dissolution. Then the water with the drug is administered. Effervescent formulations of ibuprofen commonly use a carbonate to assist in the liberation of carbon dioxide, such as Anadin LiquiFast 200mg Effervescent Tablets 16. Effervescent formulations can be prepared in two ways: direct compaction or compaction through granulation 11. They are produced in the same manner as conventional tablets; however production must occur in low humidity areas 17. Examples of ibuprofen effervescent tablets include Advil, Ibuprofen losan effervescent tablet etc. Gel is a semi-solid, topical formulation. It is formed by aggregation of particles and interpenetrated by a liquid. The particles are linked together forming a network thus imparting rigidity to the structure. The continuous phase is held together by meshes 11. Gels tend to be epicutaneous, it is directly applied to the skin, and works by diffusing through the skin. There is a liquid phase that may be retained within a three dimensional polymer matrix. Drugs can be suspended in the matrix or dissolved in the liquid phase. They tend to be aqueous gels that is applied to the body surfaces such as skin or used as lubricant. A few advantages of gels include it avoids drug absorption in the gastrointestinal, therefore reducing side effects. It avoids first pass metabolism suggesting more drug is present in the systematic circulation. They are cheap to manufacture and have a localised effect, hence greater pain relief 12. Examples of ibuprofen gels include Ibuleve, Ibugel etc. A suspension is a coarse dispersion of sparingly soluble or insoluble drugs dispersed in a liquid medium; oily or aqueous vehicle. The aqueous solution is a beneficial formulation as it provides administration of poorly soluble or insoluble drug. As the drug is dispersed, it provides a large surface area which ensures high bioavailability for dissolution thus absorption 11. Aqueous suspensions can be used for oral, topical, ophthalmic and parenteral administration of drugs. The rheological properties are affected by the degree of flocculation. This is because the quantity of free continuous phase is decreased as it is entrapped in the diffused follicles 11. From Figure 5 9, the process involved in the formation of suspensions can be seen. The flocculated state (C) can be reached directly or indirectly. The direct method includes wetting and dispersing of hydrophobic particles (A) with a surfactant. The indirect approach includes first wetting and dispersing to form a peptized particle (B) with a surfactant, and then flocculating with a hydrophillic colloid. Flocculated suspensions (C) are considered stable, compared to peptized particles, and they can be re-suspended through agitation. Over-flocculation can be caused through high amount of flocculating agent, which tends to cause agglomeration (E). If the protective colloid agent is not present, the process of crystal growth is indicated by the arrow connecting (A) to (D) 9. Examples of ibuprofen suspensions include Nurofen, Calprofen etc. Chewable tablets, as the name suggests is placed in the mouth and chewed. Thus the tablet is mechanically disintegrated in the mouth. However, the drug is dissolved in the stomach or intestine once swallowed and not in the mouth. This formulation is intended so the drug is immediately released, just like effervescent tablets 11. They also have similar composition to conventional tablets, apart from disintegrant is not present. This formulation can be useful as many patients (e.g. elderly) have difficulties swallowing tablets, therefore this can be an alternative dosage form. It can also be administered without the aid of water. It also complies with patient compliance. Examples of ibuprofen chewable tablets include Motrin, Advil etc. Comparison of conventional tablet and effervescent formulation In a report comparing the antinociceptive effect of both conventional and effervescent tablets, it was discovered that the mean plasma concentration of ibuprofen effervescent formulation was far greater than the conventional tablet 60 minutes after intake. This showed that more API (ibuprofen) is present in the blood, thus more drug is being absorbed by the body in the effervescent formulation than in the conventional tablet. This showed that the effervescent formulation produced a faster pain relief as it had a faster onset of action. The effervescent dosage form also appears to have a more consistent effect on intensity estimates of painful stimuli than tablets 18. The chemo-somatosensory event related potentials were also investigated and it was concluded that after 60 minutes of administration of the ibuprofen tablet, there was a decrease of 20-25% in bioavailability. As there was a large drop (a quarter of the bioavailability), it shows there is 20-25% less ibuprofen present in the blood thus less API is being absorbed. This means a higher dose of ibuprofen tablet (20-25%) is required to have the same effect as initially thought 18. The effervescent formulation is far more effective in terms of chemo-somatosensory. This is because greater amount of carbonate is present; therefore after dissolution a buffered solution is attained. This increases the pH of the stomach resulting in the emptying of the stomach at a rapid rate and the residence time of ibuprofen in the stomach is short. This ensures that ibuprofen-induced gastric irritation and other side effects can be avoided 11. Ibuprofen effervescent tablet is readily absorbed in the small intestine; ensuring fast drug bioavailability 11. In another report, Lange and Schettler showed that effervescent formulations of ibuprofen produced a higher maximum plasma concentration (Cmax) than the conventional tablet in a shorter duration. The same as the previous report 19. In respect to antinociceptive and chemo-somatosensory activity, the effervescent formulation is superior to the conventional tablet. To consume the ibuprofen tablet you require water to administer it. While with the ibuprofen effervescent formulation it requires water so the tablet can disintegrate. Effervescent formulations also have to be manufactured at a low humidity area compared to the conventional tablets. This is to avoid moisture content, light and oxygen and this procedure is more costly than the conventional tablet. Effervescent tablets also have to be packaged in waterproof containers which have aluminium foil present which ensures protection, otherwise in ambient conditions it would degrade and reduce the shelf-life 11. If the shelf-life is reduced this would increase the cost, as more effervescent tablets would have to be manufactured over the same duration. An example of an effervescent tablet is Anadin LiquiFast 200mg Effervescent Tablets (Figure 6) 20 and a conventional tablet is Nurofen Tablets (Figure 7) 21. Figure 6: Shows Anadin Effervescent Tablets Figure 7: Shows Nurofen Tablets Comparison of conventional tablet and a topical gel There was a report comparing the oral (tablet) and topical (gel) ibuprofen for chronic knee pain. Ibuprofen tablets were taken 3 times daily (2400mg total) and the ibuprofen gel 4% was applied 4 times daily (320mg total) over a duration of 2 weeks 22. The aim was to compare the efficacy of both formulations in chronic knee pain. Both the treatment groups were comparable in terms of baseline pain severity and demographic composition, this ensured a fair test. When the patients took their respective ibuprofen medications, they reported side effects. With the administration of the tablets, 7 patients (out of 10) reported side effects which included headache, stomach-ache and constipation. For the application of the gel, 2 patients (out of 9) suffered a side effect which included an acute skin rash and dizziness. This implies that over two-thirds of the patients taking the oral formulation reported a side effect, and for the topical formulation less than third reported a side effect. As a result there were fewer side effects associated with the topical formulation compared to the oral formulation 22. In both treatments, the patients experienced consistent relief and improvements in terms of pain and stiffness. There was no distinguishable difference between both groups in term of improvements. Both the treatment groups were similar and no group was better than the other. However, the oral group ranked their treatment more convenient as it met patient adherence. Comparing the physical function and relief of pain and stiffness, it shows the oral ibuprofen treatment saw notable improvements. In the topical ibuprofen treatment there were significant improvements over the two week duration. However for the oral treatment there was a decline in the improvement of the drug in the second week. Also in the topical treatment, the patients encountered within-group improvements which led to the assumption that it was due to the potential benefits of massaging. The topical ibuprofen was applied to the skin therefore there was less amount of drug was present in the blood compared to ibuprofen tablets. This avoided both the systemic side effects and adverse drug interactions (e.g. aspirin), unlike the oral ibuprofen 22. The total daily dose of the topical ibuprofen was 320mg, which is a small fraction compared to the oral ibuprofen (2400mg). However, similar clinical outcomes were produced using both treatments. Another report concluded a study where the patients were receiving equivalent doses of oral and topical formulations. During the topical application greater concentrations of ibuprofen were found in the subcutaneous tissue, which led to the assumption as more ibuprofen is present in the tissue; it is able to provide greater pain relief 23. In terms of physical function, pain and stiffness relief, the topical formulation is superior to the oral formulation. An advantage of topical ibuprofen over ibuprofen tablets is first pass metabolism is bypassed therefore it avoids risks and unwanted effects. Topical ibuprofen is a transdermal delivery system, which is more efficient than the oral delivery due to having an effect at a localised level. Comparison of conventional tablet, chewable tablet and suspension There was a report comparing the pharmacokinetic parameters of ibuprofen tablets, ibuprofen chewable tablets (Motrin chewable tablets) and ibuprofen suspension (Motrin suspension) on patients with cystic fibrosis. This study is limited as the number of patients taking each formulation is different; suspension (n=22), chewable tablets (n=4) and tablet (n=12). , nor is the strength or concentration given for any of the formulations. However patients took a dose of approximately 20mg/kg. The time to reach the peak concentration (Tmax) was compared for all formulations and it was concluded that the ibuprofen suspension had a shorter Tmax than the ibuprofen tablet, which was expected when liquid forms are compared with solid dosage forms. But with the ibuprofen chewable tablet there was no statistical difference from either the tablet or suspension. This was not expected as it was predicted the chewable tablet would have a greater Tmax to suspension but less than that of tablets. This is because chewing the chewable tablet produces small particles hence larger surface area, which should increase the dissolution of the drug 24. As the Tmax for suspension was shorter than the tablet it can be concluded more drug was present in the body at a quicker rate, hence more drug would be absorbed thus causing faster pain relief. From Figure 8 24 and Table 2 24, it can be concluded that 15 of the patients taking the suspension formulation were present in the desired range of peak concentration (therapeutic range), and this was achieved at a quicker rate of ~0.5 hours compared to the other formulations. For the chewable tablet it can be seen that 2 of the patients consuming it were in the therapeutic range, however it took patients to reach this level between ~1.0-2.0 hours. For the patients who consumed the tablet, 8 patients were in the therapeutic range. However it took between ~0.75-2.0 hours to reach to this level. There was also one anomaly present In the suspension group 5 patients, and 2 patients from the tablet groups exceeded the therapeutic range (>100mg L-1), therefore they are more prone to experience side effects. None of the patients who consumed chewable tablets exceeded the therapeutic range. For all formulations, 2 patients were below the therapeutic range ( It can be concluded statistically tablets were superior in terms of achieving the desired range of peak concentration compared to other formulations. But statistically suspensions were superior in terms of the time taken to reach the therapeutic range compared to both formulations. From Figure 9 24, it can be seen patients taking the suspension reached max plasma concentration and at a quicker duration compared to other formulations. It took 0.5 hours to reach a plasma concentration of 70mg L-1, while tablets took 1.0 hour to reach a plasma concentration of 60 mg L-1. The chewable tablet took 1 hour to reach 50mg L-1 (Cmin). Also for the suspension, between 0.5-1.0 hours it remained in the therapeutic range. For the tablet, the period between 0.75-1.0 hours it remained in the therapeutic range. It took the chewable tablets 1 hour before it reached the therapeutic range, before and after this period it had little effect. As the time reaches 6 hours, the suspension had the lowest plasma concentration, while the tablet had the highest concentration. However it was below the therapeutic range therefore at this point all formulations have no effect 24. It can be concluded ibuprofen suspension is the superior formulation in terms of pain relieving as it reached the highest plasma concentration at a quicker duration, and was in the therapeutic range for the longest period. It is closer to Cmax at 70 mg L-1, while the other formulations were below this. Due to these factors it can be seen that the suspension has greater amount of ibuprofen present in the blood, hence more drug is going to be absorbed in the body. It includes drugs that are efficacious, safe and cost effective for conditions that are a priority. These conditions are selected on the foundation of current and future public health relevance. In terms of pain relief, antinociceptive and chemo-somatosensory effect, ibuprofen effervescent tablets are far superior to ibuprofen tablets. They caused faster pain relief at a quicker rate. Both ibuprofen gel and ibuprofen tablet produced similar efficacy in terms of pain relief in their respective groups. However, the dose administered in the gel dosage form was four times less than that of the ibuprofen tablet. Fewer side effects were observed as it avoided gastric irritation, in comparison to ibuprofen tablet. Ibuprofen tablet caused gastric irritation, which can possibly lead to liver and kidney damage. Patients consuming ibuprofen suspension reached the therapeutic effect at a quicker rate and were in that period for a longer duration compared to the other dosage forms. This suggested that it caused faster pain relief for a longer duration compared to ibuprofen tablets and ibuprofen chewable tablets. Ibuprofen chewable tablets reached a maximum concentration of 50mgL-1, so it just reached the base of the therapeutic effect, which suggests it has poor efficacy.

Although forty years have gone by since Title IX was initially passed, women are still fighting a daily battle for equality in the employment field. Women have made great strides in terms of the careers they choose to pursue: â€Å"According to the Women’s Bureau (2006), 118 million women comprised 46% of the total U.S. labor force† (Chavez et al 79). However, employment is still not completely unbiased and equal between males and females and much of the inequality is due to subjective terms of leadership abilities. Research has shown that males and females have developed different leadership styles, and one may be more preferable than the other for high level corporate positions. According to authors Esther Chavez, Mark Green, and Diana Garza-Ortiz, leadership can be defined as â€Å"the process whereby an individual influences a group of individuals to achieve a common goal† (Chavez et al. 79). By this definition, any person can be a leader regardless of gender, age, and other stereotypical constraints. â€Å"Euthanasia is the practice of ending the life of a patient to limit the patient’s suffering. The patient in question would typically be terminally ill or experiencing great pain and suffering. The word â€Å"euthanasia† itself comes from the Greek words â€Å"eu† (good) and â€Å"thanatos† (death). The idea is that instead of condemning someone to a slow, painful, and undignified death, euthanasia would allow the patient to experience a relatively good death.† The technical definition of euthanasia is the act of ending life painlessly, often someone suffering from an incurable illness. However it is impossible for any life to end free from pain. The actual killing may be peaceful, but the suffering endured throughout the disease will never be forgotten and the heartbreak felt by the family due to the untimely death of a loved one will live on forever. Euthanasia is an extremely controversial issue dividing professionals in both the medical and legal fields. Some argue that individuals have a right to die and death is a choice. Those against euthanasia argue that, â€Å"Death occurs because a fatal pathological condition is allowed to take its natural course, not because those who have removed life support intended to kill the patient. Rather, their intention is to stop doing something useless or to stop imposing a burden on the patient.† (Yount 23) The Netherlands, Belgium, and the Northern Territories in Australia are among some of the countries that recognize Euthanasia as legal. In many cases complications occur such as vomiting and intense muscle spasms. Also in a significant number of cases the drugs provided did not result in the desired outcome, meaning then the physician has to inject other fatal concoctions by hand, breaking their Hippocratic Oath. If euthanasia becomes completely legal it will allow a price to be placed on human life, and provide the mentally ill with a legal outlet to give into their disease. Even though a life stolen by euthanasia is supposedly painless, the pain endured by a premature death will live on in the hearts of their loved ones forever.
